Exempt doesn't mean the odometer doesn't work. They do that on older cars usually. When I bought mine, the title was from the original owner, when I got my title in the mail, it said exempt on the odometer even though I'm the 2nd owner and the speedo hasn't been tampered with.
As for starting it... Do a fluid change on anything with fluid. Replace the old gas with new gas, there's some other things that need to be done as well to "prime" it, but I'm not a tech so not really sure on what all is involved. You'll probably want to change the fuel filter as well, maybe twice after you drive it a while, just to make sure there's no gunk in the fuel line from the tank to the filter.
