You can make one if your handy enough. Spectre makes a kit with elbows and straight pieces that can be cobbled together. Even includes pieces with pre-fabbed holes for the various air lines you mentioned.

A couple of key points. All the air lines that are stock-located after the MAF sensor must remain after the MAF sensor. Capping them off is a bad idea, and leaving them open will confuse the ECM. Any air leaks between the MAF sensor and throttle body will also confuse the ECM, so make sure it all seals well.
The purpose of a CAI is to draw cold air. So, you either need to box in the filter or run it out of the engine bay and away from engine heat. I've seen these run into the inner fender area behind the inner fender liner and in front of the radiator. Just make sure you don't run it too low. It would suck to hydrolock the engine hitting a puddle at speed.
